Preferrably, input your first name, middle initial and last name as your Username or a variation of that.
Examples: Micah R Johnson, Micah R. Johnson or MicahRJohnson

A valid e-mail address. All e-mails from the system will be sent to this address. The e-mail address is not made public and will only be used if you wish to receive a new password or wish to receive certain news or notifications by e-mail.

You will receive an e-mail notification containing a temporary password for a one-time login. After logging in, you will be able to change your password.
